Each of the forms provided on the Court Documents and Bail Documents pages are in a fillable PDF format. This means that certain sections of these forms have been set up so that you can type your responses into them prior to printing them.
SIDE NOTE: The light-colored boxes that show up on the fillable PDF forms do not show up when the form is printed - only what you've typed in will show.

Here are some instructions about using these forms:
- When you click on one of the forms, it will open in a new tab. You may customize the form directly from that new tab and then print it. You may also download a copy of the form.
- It is suggested that you download and save a copy of the forms because:
- When you download a copy, you can then create a template version which is specific to your needs. For example, you can save a copy with fields pre-filled that remain the same every time it's used (i.e., your name, your company, your address, etc.). Use this new template from that point on to save a bit of time in the future.
- A downloaded copy of the form will give you full functionality of the fields you will be filling out (this isn't always available in the online version of the form). For example, date fields in the forms have been formatted so that a calendar box is available to select a date (via a drop-down menu on the right side of the date field). You cannot do this with the online version of the form.
- To begin filling out the form, click in the shaded field which needs to be filled and then type in your information. You can move to the next field easily with the Tab key (or go backwards to previous fields by pushing the Tab + Shift keys at the same time).
- You do not need to type a $ sign when filling out fields involving money. Fields containing money amounts have been preformatted so that when you enter an amount, the form will automatically format it in the correct currency style. For example, if you type 1000 it will turn into $1,000.00 automatically.
- When you are entering dates there are a few options. The fields have been formatted so that dates will show up like this: 1/1/19. In this example, you can enter it in one of the following ways:
- Select the date you need from the drop down calendar by clicking on the down arrow to the right of the entry field.
- Enter the date like this and let the PDF automatically format it: 1-1-19.
- Enter the date like this: 1/1/19.
- With phone numbers, you only need to enter the digits themselves and it will turn into standard phone number format when you move to the next field. For example, 6015551234 will automatically turn into (601) 555-1234.
- Social security numbers have also been preformatted. All you need to do is enter the number (without spaces or dashes) and the form will format it automatically in the standard SSN format (123-45-6789).
